Introduction
These Terms and Conditions ("Terms") govern your use of the Pilothouse Health Remote Therapeutic Monitoring (RTM) platform and your receipt of SMS and email communications from Pilothouse Health. By enrolling in the program, you agree to these Terms.
Please read these Terms carefully. If you do not agree, please contact your medical provider to discuss alternative care options.
The RTM Program
Pilothouse Health provides a fully managed Remote Therapeutic Monitoring platform on behalf of your medical provider. The program allows you to submit regular health logs that your care team reviews between visits.
Your participation involves:
- Completing short weekly health logs via the Pilothouse Health patient portal
- Receiving weekly SMS and email reminders to submit your log
- Having your health data reviewed by your assigned provider and care team
Enrollment and Consent
You are enrolled in the Pilothouse Health RTM program by your medical provider. By completing your account setup and submitting your first health log, you confirm that you:
- Are 18 years of age or older, or have parental or guardian consent
- Consent to receive weekly SMS reminders from Pilothouse Health
- Consent to have your health log data shared with your assigned medical provider and care team
- Have read and agree to these Terms and our Privacy Policy
SMS Terms
Program description: Pilothouse Health sends weekly SMS reminders to enrolled patients to submit their health log via our secure patient portal.
Message frequency: Approximately one message per week.
Message and data rates may apply. Standard messaging rates from your mobile carrier may apply to messages you send and receive.
Supported carriers: AT&T, Verizon, T-Mobile, Sprint, and most major US carriers. Carrier support is not guaranteed for all carriers.
To opt out: Reply STOP to any SMS at any time. You will receive a final confirmation message and no further SMS messages will be sent. Opting out of SMS does not unenroll you from the RTM program — you can still access the patient portal and submit logs directly.
To opt back in: Reply START at any time to resume SMS reminders.

Patient Responsibilities
As a participant in the RTM program, you agree to:
- Submit health logs honestly and to the best of your ability
- Keep your contact information current by notifying your medical provider of any changes
- Use the patient portal only for its intended purpose
- Not share your account credentials with anyone else
- Notify your care team of any urgent medical concerns directly rather than through the health log
Important
The Pilothouse Health platform is a monitoring and communication tool. It is not an emergency service. If you are experiencing a medical emergency, call 911 immediately. Do not use the patient portal or SMS system to report emergencies.
Not a Substitute for Medical Care
The Pilothouse Health platform supports your care between visits but does not replace your relationship with your medical provider. Health log data is reviewed by your care team as part of your ongoing care, but submission of a health log does not guarantee an immediate clinical response.
Your provider determines how and when to act on RTM data as part of their clinical judgment.
Intellectual Property
The Pilothouse Health platform, including its software, design, and content, is owned by Pilothouse Health and protected by applicable intellectual property laws. You may not copy, modify, distribute, or reverse engineer any part of the platform.
Limitation of Liability
To the fullest extent permitted by law, Pilothouse Health shall not be liable for any indirect, incidental, special, or consequential damages arising from your use of the platform or participation in the RTM program.
Our total liability to you for any claim arising under these Terms shall not exceed the amount paid by your medical provider for RTM services in the month in which the claim arose.
Termination
Your access to the Pilothouse Health platform may be terminated:
- By your medical provider, if you are discharged from their practice or unenrolled from the RTM program
- By Pilothouse Health, if you violate these Terms
- By you, by contacting your medical provider or Pilothouse Health at team@pilothousehealth.com
Termination does not affect any rights or obligations that arose before termination.
Changes to These Terms
We may update these Terms from time to time. We will notify you of material changes by posting the updated Terms on our website. Your continued use of the platform after changes are posted constitutes your acceptance of the updated Terms.
Governing Law
These Terms are governed by the laws of the State of California, without regard to its conflict of law provisions. Any disputes arising under these Terms shall be resolved in the courts of Contra Costa County, California.
